Download the appropriate client form and return it below before your appointment. You can also come 30 minutes prior to your first appointment and fill out the paperwork in the office lobby if you do not have access to a printer.
I enjoy meeting new clients, so feel free to visit during my lunch break, between noon and 2 PM during business hours. Please call before coming so I can meet you in the lobby. While I am available for consultation and questions, on Thursday I do not see clients for sessions.
